I'd say the primer shoved the bullet to the barrel throat...

Bullet is partially in the cylinder and partially in the barrel.

Guy brought me one like that once. I used a wooden dowel and knocked the bullet back into the cylinder and all was well.

Take that for what it's worth. May not be the same issue, but bet it is.
