dirt and residue holding the breach open a little bit or perhaps a very weak recoil spring allowed the bolt to open a smidge premature, with pressure still high.
I've read that some early Colt .22's did NOT have recessed rimfire chambers and they cautioned against hi-speed .22 ammo. Now days most rimfire revolvers still have recessed chambers. I guess not too bad an idea on any rimfire. Centerfires with recessed chambers (like my M27 S&W) make me remember back in the day they had balloon case head brass. I guess they could let go under the right circumstances.
