several different things that could do that but it is a finite collection. An "underload" of a slow powder, if proper slow powders are used you can't put enough of it in to blow up! But an underload can do it. Pistol powder comes to mind immediately.

Accidents can happen but I certainly doubt a design issue or issues related to the bullet. If it was too big for the bore it would not fit into the chamber to fire. If you look at the case it blew about half-way up. I think that might be a clue.
