When I started experimenting with heavy 300 gr. loads in the Ruger Blackhawk I went through that and figuring out how to keep it on.
Also the firing pin bushing in some of the early Rugers were soft. In my gun, with high pressure loads, the bushing was set back into the frame, and pounded in around the firing pin hole until the firing pin stuck.
I case hardened a new bushing and shimmed it so that it could not back up into the frame.
The recoil impulse of heavy loads stresses most everything.
