the Medical Directive includes a DNR, Medical Power of Attorney which you designate those you trust to make the choices you have expressed, Durable Power of Attorney giving those you trust ability to take care of bills, etc, HIPPA Release so the Doctors can give them all the medical stuff they have about you, and a Directive to Physicians expressing your desires about end of life.
